Faith Clause, FNP-C is a certified Family Nurse Practitioner and is the co-owner and strategist behind Transform Health & Aesthetics. Her training encompasses years of strategic medical treatment in neurology and family medicine which is how she developed expertise in medical weight loss and years of injecting skills in Botox. Growing a holistic concierge medical practice and spa that caters to promoting wellness is her dream and mission.

Cheryl Wiley, FNP-C is certified Family Nurse Practitioner and co-owner of Transform. Cheryl has over 20 years of experience as a nurse practitioner. She has worked in the Emergency Department, Family Practice, and the last 13 years as a Women’s Health Provider in a busy OB/GYN practice. Cheryl was born and raised in Redding and is a very active member of this community. She is happily married for over 25 years now and has three daughters. Cheryl is dedicated to providing quality, compassionate and holistic healthcare in our community.

Dawnya Jones, RN is an Registered Nurse with a wealth of experience in patient care. She comes to Transform with a passion for holistic health care, getting to the root cause of medical needs and finding solutions to tough and challenging problems. Her work ethic and spirit of excellence allows her to shine. She was raised in the Shasta County area and lives here locally with her spouse and three boys.
